Women and Medicine: Why We React Differently Than Men – and Why It Matters

For decades, medical research has been built around the male body. That legacy still shapes how women are diagnosed, treated, and experience side effects today. We now know that sex plays a far greater role in how the body responds to medication than once believed. But why do these differences exist—and why is it so important to take them seriously?
A Historical Bias in Research
Until the early 1990s, women were often excluded from clinical trials in the United States. Researchers worried that hormonal fluctuations, pregnancy, or “too many variables” would complicate results. As a result, drug dosages, side effects, and safety data were largely based on studies of men.
It wasn’t until 1993 that the National Institutes of Health (NIH) required federally funded clinical research to include women. Even so, the effects of decades of male-centered research remain. Many drugs are still not adequately tested on women, and women continue to experience stronger or unexpected side effects more often than men.
Biological Differences That Matter
Men’s and women’s bodies differ in ways that go far beyond reproductive organs. Variations in body composition, enzyme activity, fat distribution, and hormone cycles all influence how drugs are absorbed, metabolized, and eliminated.
- Hormonal fluctuations: Estrogen and progesterone affect both the immune system and how the liver processes medication. This means that the effectiveness or side effects of certain drugs can vary throughout a woman’s menstrual cycle.
- Body fat and weight: Because women generally have a higher percentage of body fat, fat-soluble drugs can accumulate differently, altering how long they stay in the body.
- Liver enzymes: Women often have lower activity of certain liver enzymes that break down drugs, which can lead to higher concentrations in the bloodstream and a greater risk of side effects.
These differences mean that a “standard dose” designed for a man may not be the right dose for a woman.
When Symptoms Look Different
The differences don’t stop with how drugs work. Women and men can also experience diseases differently. A well-known example is heart attacks: while men often report chest pain, women are more likely to experience nausea, fatigue, or pain in the back or jaw. For years, these subtler symptoms led to misdiagnoses and delayed treatment for women.
Gender differences also appear in mental health, autoimmune diseases, and pain management. Women are more likely to suffer from depression and anxiety but may respond differently to antidepressants. They are also more prone to chronic pain conditions—yet often receive less effective pain relief.
The Cost of Ignoring Sex Differences
When medicine fails to account for sex differences, the consequences can be serious. Women are more likely to experience adverse drug reactions, be misdiagnosed, or receive treatments that don’t work as intended. This affects not only quality of life but also survival.
A striking example is the sleep medication zolpidem (sold under brand names like Ambien). After years on the market, researchers discovered that women had nearly twice the blood concentration of the drug as men after taking the same dose. In 2013, the FDA finally recommended cutting the dose for women in half. It was a wake-up call for the medical community—and a reminder that “one size fits all” medicine can be dangerous.
The Future of Gender-Specific Medicine
Today, awareness of sex differences in medicine is growing. The NIH and the Food and Drug Administration (FDA) now require that both women and men be included in clinical trials and that results be analyzed by sex. At the same time, the rise of precision medicine—which tailors treatment to an individual’s biology, including sex and genetics—offers new hope for more effective care.
Still, there’s a long way to go. Many physicians receive little training on how sex differences affect diagnosis and treatment, and healthcare systems often rely on outdated standards. Addressing this gap requires not only better research but also education, awareness, and advocacy.

As a patient, you can play an active role. Ask your doctor whether sex differences are known for the medications you’re prescribed. Pay attention to side effects and report anything unusual. The more data we gather, the better treatments can become for everyone.
Recognizing sex differences in medicine isn’t about dividing people—it’s about achieving equality. Only when we understand how women’s and men’s bodies respond differently can we ensure that every patient receives the safest and most effective care possible.
